High School/Adult School Counselor Referral

Please use this form to submit a referral for a Dual Enrollment student taking classes at CRC.  If possible, please provide insight to how to best connect with your student and inform the student of the referral ahead of time. We will do our best to connect with the student in seven business days.

Note: The purpose of this form is not to request the academic standing of a dual enrollment student.
