Transcript Notation Request Form

If you would like your ASCRC involvement noted on your official CRC transcript, then you must fill out this form and submit directly to the Advisor no later than 30 days prior to the end of the spring semester. The transcript notation will only be on your transcript once per academic year after the spring semester has ended.

All requirements must be met in order to have "Recognized Student Government Officer" noted on your transcript. If you have questions or need additional information, please contact the Advisor directly. Thank you.
