As a faculty member, staff member, or student, if you are concerned about a student's behavior please fill out the form below.

Please be assured of the following:

• All reports are confidential
• You will receive acknowledgement that the report has been received and that appropriate action will be taken

Please note:

• Please inform the student a referral has been made and the student may be contacted by a BIRT team member                       
• All mental health services will be provided remotely until further notice
