This is a Mandatory Support Check-In that you must complete for failing to meet units and GPA requirement.
Important information:
- Students must complete this form as part of the petition process
- After submitting the form, you will be contacted to schedule a 1 hour appointment via ZOOM or in-person with an EOPS counselor.
- If you do not complete both form and appointment for the Mandatory Support Check In, you will not receive any EOPS incentives which includes the parking permit, the EOPS book grant, and the EOPS Contact(s) incentives Spring 2025.
