This is a Mandatory Support Check-In that you must complete for failing to meet units and GPA requirement.
Important information:
- Students must complete this form as part of the petition process
- After submitting the form, you will be contacted to schedule a 1 hour appointment via ZOOM or in-person with an EOPS counselor.
- If you do not complete both form and appointment for the Mandatory Support Check-In, you will not receive any EOPS semester support which includes the parking permit, the EOPS book grant, and the EOPS Contact(s) semester support for Fall 2026.
